I read this on Wednesday morning on Bleacher Report. It's very sad. He was a tremendous character. Big part of my childhood.
Unfortunately he had a lot of issues with his weight, during his hiatus from WWE he was 500lbs and he as you know was a very short man.

Especially for a very short man. He lost some weight when he came back for a brief stint but he looked incredibly weak and unhealthy

Like a lot of WWE heroes who died too young the constant touring non stop is very hard on them.
Maybe not so relevant to Paul Bearer but there is also a lot of pressure on the older men to stay fit and capable to run with the new blood.
Men like Eddie Guerrero are testament to this imo.

he towered at 7ft 4inch and his shoe size was size 16

At the wrestling last year, after a few cheeky pints I might add, I stood up to the Great Khali and told him I'd take him on!
